Family Night Transportation Permission

*Required for children 4 to 18 years of age to receive transportation services to Family Night and/or the Christmas Program when attending events without adult accompaniment.

2025/2026 Family Night – 5:45pm

September 28, October 26, November 9, December 14 (Christmas Program @5:30pm), January 25, February 22, April 5 (Easter Dinner), April 26 , May 31, June 28

Note: In addition to the completed permission form, we ask that you fill out the “Request a Ride” form for each family night attended.
